Evercare was started in 1987 by two Nurse Practitioners in Minnesota. We have grown from a small Medicare pilot program to an international, award-winningA health care organization. Today, Evercare serves thousands of members across the country.

Evercare health plans are offered by United Healthcare Insurance Company, who contracts directly with Medicare and several state governments to provide health care services to both Medicare and Medicaid recipients in their communities. In most cases, we are able to provide our members more benefits and coverage than Medicaid and Medicare alone.
We offer Medicare Advantage Plans, which provide Medicare Part A and Part B coverage, plus additional benefits and services not covered by Original Medicare.
For members who live in nursing homes, a nurse practitioner or a physician assistant and a primary physician form a primary care team. Together, they work with other health care providers and nursing home staff to help maintain the health and well-being of our members. We also work with members who live in the community to help ensure optimal use of available health care resources. For these individuals, a Care Manager works with the primary physician, other health care providers and local service organizations to help coordinate and gain access to community-based assistance.
