Evercare Medicare Advantage plans are part of the Medicare program. These plans help you access Medicare benefits and provide you with personalized, coordinated care to help keep you healthy and as independent as possible. Evercare offers three different types of Medicare Advantage Plans.

Health Plans for People with Limited Incomes
These plans are designed for people who are both Medicare and Medicaid eligible, or "dual eligible". These plans include all Medicare Part A (Hospital) & B (Doctor) benefits and Part D prescription drug coverage. For people with limited incomes, these plans may offer better health care coverage than Original Medicare and a separate Part D plan.

Health Plans for People with Specific Long Term Illnesses
If you or your loved one has a severe or disabling chronic (long-term) condition, such as cardiovascular disorders, congestive heart failure or diabetes mellitus, we have special Medicare plans designed for you.

Health Plans for People in Nursing Homes
This Evercare plan helps nursing home residents get access to personalized, coordinated care.

Coordination of Long-Term Services (CoLTS)
Evercare provides a state-specific health plan for Medicaid recipients who live in New Mexico called Coordination of Long-Term Services (CoLTS). This plan provides coordinated, personalized care to help you use your benefits to stay healthy.

Evercare™ Solutions for Caregivers
Evercare offers a nationwide service that helps caregivers and their loved ones with needed care for aging, disabled or chronically ill loved ones.
