Medicare Advantage Plans
Medicare Advantage Plans are health plan options that are approved by Medicare and run by private companies. They are part of the Medicare Program, and sometimes called "Part C." When you join a Medicare Advantage Plan, you are still in Medicare.

All of our Medicare Advantage Plans come with Part D prescription drug benefits. These plans help protect you from the rising costs of medications you may need to stay healthy. For more information about Medicare prescription drug benefit, go to our introduction to Part D.

Health Plans for People with Limited Incomes
If you need to stretch your health care dollar, Evercare provides a number of state-specific health plans for people enrolled in Medicare and who are eligible for Medicaid benefits (or "dual eligible"). These plans provide access to coordinated, personalized care to help you use your benefits to stay healthy.
Health Plans for All Medicare-Eligible People
HMO Medicare Advantage plans for people with Medicare. These plans are geared for people who live in their own home, a retirement community orassisted living facilities. Plans are available either with or without included Part D Prescription Drug coverage.
Health Plans for People in Nursing Homes
Evercare helps you access personalized, individualized, coordinated care for nursing home residents. Evercare supplements nursing facility services with an added layer of care, more covered benefits and a Nurse Practitioner for no extra monthly health plan premium.
Health Plans for People with Specific Long Term Illnesses
If you have a cardiovascular disorder, chronic heart failure or diabetes, Evercare can help you manage not only your illness, but also your overall health and well-being.
