Evercare Senior Care Options (Evercare SCO)

Evercare Senior Care Options (Evercare SCO) is a coordinated health care plan that is offered by United HealthCare Insurance Co., an organization that contracts with the Federal government and with Massachusetts Medicaid to deliver and coordinate all Medicare and Medicaid covered benefits for eligible individuals. Our goal is to keep elders as independent as possible.

We offer broader health care coverage than Original Medicaid or Medicare alone including:

• A Care Manager to coordinate the enrollee's medical, home care and other services
• No copays on covered drugs from our contracted pharmacies
• Personal care, housekeeping, chores (if medically necessary) and transportation (if the enrollee needs a ride to the doctor, clinic or hospital)
• Annual physical exam
• Preventive services
• Routine eye exams, glasses and corrective lenses once a year
• Preventative dental care including routine check-ups and cleaning every 6 months (dentures are covered once every five years or more often if medically necessary)
• Hearing tests once a year and hearing aids every 5 years or more often if medically necessary
• Chiropractic services
• Routine foot care for members without a medical condition affecting the lower limbs, limited to 6 visits each year
• Prosthetics and orthotics
• Nursing Facility Coverage
• Community long-term care services
• Interpreter services

Evercare Senior Care Options is available to people who:

• Are at least 65 years old
• Have MassHealth Standard coverage
• If you have Medicare, you may have either Parts A or B or both
• Live in the Evercare SCO service area
• Do not have End Stage Renal Disease (ESRD)
• Do not reside in an intermediate care facility for the mentally retarded (ICF/MR)
• Are not on hospice care, unless the hospice provider is part of our Contracted network of providers
• Agree to follow Evercare SCO rules and seek treatment from our Contracted Doctors and health care providers
• Pay any patient paid amount or contribution to care to us to retain your MassHealth Standard coverage if required

Evercare Senior Care Options is available to MassHealth Standard (Medicaid) recipients residing in Bristol, Essex, Hampden* (partial county including Agawam, Bondsville, Brimfield, East Longmeadow, Feeding Hills, Hampden, Holland, Holyoke, Longmeadow, Monson, Palmer, Springfield, Thorndike, Three Rivers, Wales, West Springfield, Wilbraham, Middlesex, Norfolk, Chicopee, Holyoke, Ludlow, Woronoco), Plymouth* (partial county including Marion, Mattapoisett, Abington, Brant Rock, Stoughton, Bridgewater, Brocton, Bryantville, Duxbury, East Bridgewater, Elmwood, Green Harbor, Halifax, Hanover, Humarock, Kingston, Marshfield, Marshfield Hills, Monponsett, North Marshfield, North Pembroke, Pembroke, Plympton, Rockland, West Bridgewater, Whitman, Accord, Greenbush, Hingham, Hull, Minot, North Scituate, Norwell, Scituate), Suffolk and Worcester* (partial county including Linwood, Northborough, Southborough, Westborough, Auburn, Barre, Boylsoton, Cherry Valley, Fiskdale, Gilbertville, Grafton, Hardwick, Holden, Jefferson, Leicester, Manchaug, Millbury, New Braintree, North Grafton, Oakham Rochdale, Rutland, Shrewsbury, South Barre, South Grafton, West Boylston, Wheelwright, Whitinsville, Wilkinsonville, Worchester, Ashburnham, Baldwinville, Berlin, Bolton, Clinton, East Princeton, East Templeton, Fitchburg, Gardner, Hubbardston, Lancaster, Leominster, Lunenburg, Princeton, South Lancaster, Sterling, Still River, Templeton, Westminster, Winchendon, Winchendon Springs, Blackstone, Brookfield Charlton, Charlton City, Dudley, East Brookfield, East Douglas, Hopedale, Mendon, Milford, Millville, North Brookfield, North Oxford, North Uxbridge, Northbridge Oxford, Southbridge, Spencer, Sturbridge, Uxbridge Warren, Webster, West Brookfield, West Brookfield West Upton, West Warren) counties.

Evercare Senior Care Options is a coordinated care plan. It has a contract with Massachusetts Medicaid and the Federal government. To access benefits, you must have services provided by your primary care team. Or, you must have services approved by your primary care team. These services must be received from providers in the Evercare Senior Care Options network. Evercare is offered by United Healthcare Insurance Company (UHIC).
