The QExA program is a Hawaii Medicaid managed care program administered by the State of Hawaii, Department of Human Services, Med-QUEST Division. Eligibility for this program is determined by the State of Hawaii.

The Evercare QExA plan is one option available for eligible residents of Hawaii who qualify for the QExA program.*

What Is Evercare QExA?
Evercare QExA is a managed care program offered through Hawaii's QUEST Expanded Access. Evercare QExA is a health plan for older adults, and for children and adults with disabilities and other health care needs. With Evercare QExA, you are at the center of your health care.

Our number one goal is to help you stay healthy and independent. The Evercare QExA plan offers services to help you with daily life such as adult day care, personal help, assisted living, meals and supplies. We can even help with minor home changes so you can get around more easily. Choose Evercare QExA today to get expert service coordination.

Evercare has been doing care management for a long time.
You will work with a local service coordinator who will create a care plan to fit your specific needs. We will help guide you through the complex health care system and work with your doctors and family members to help you meet your health-related goals.

We coordinate with Medicare
If you have Medicare, we can help coordinate Medicare and Medicaid benefits and provide additional services to make your life easier. If you want, we can set it up for Evercare to take care of all your needs.

What if I am already getting care from a doctor?
Our goal is to give you a smooth transition to Evercare. We want you to keep getting good care from a provider you like. Call 1-888-980-8728 to find out if your provider is already in our group of doctors or other health care providers.

What does Evercare QExA cost?
Evercare QExA is free! There is no cost to enroll in Evercare QExA. And there are no co-payments.
