You don't have to go it alone. Evercare has a health plan specifically designed to ensure the best care possible for people living with Alzheimer's disease or a related dementia and their caregivers.

Evercare provides full Medicare coverage, including prescription drugs, and extra benefits and services tailored to address the needs of people with chronic dementia. All in one health plan.

We have designed our plan around respected and recognized guidelines for the care of dementia. Plus, we deliver our own proven Care Model to help manage other health conditions that are often present and impact the health and well-being for someone with Alzheimer's disease or related dementia.

Our plan offers a network of recognized specialists in the treatment of Alzheimer's disease and dementia, including experts from the prestigious Banner Alzheimer's Institute in Phoenix. The Institute is part of Banner Health, the region's largest non-profit health care provider. It was designed specifically to identify effective Alzheimer's disease-slowing and prevention therapies in the shortest possible time while setting a new standard of care for patients and their families.

We offer you:
  • Comprehensive Coverage
    Evercare bundles everything you need into one easy plan. We provide built-in Medicare Part D prescription drug coverage. Commonly used drugs for dementia like Aricept* are included in the formulary at a lower-cost drug tier.
  • Personalized Care
    With our special health plan, you have access to Evercare Care Managers who work with you, your family, doctors, and other care providers to help you get the care you need so you can stay as independent as possible.
  • Predictable Costs.
    Evercare plans cover all your Medicare Part A and B benefits and more, with much lower out-of-pocket costs than Original Medicare with a Medicare Supplement plan. There is a Part D monthly plan premium with low co-pays on prescriptions.

Evercare® Medicare Advantage plans are offered by United HealthCare Insurance Company and its affiliated companies, Medicare Advantage Organizations with a Medicare contract. Limitations, co-payments and coinsurance will apply.
