Evercare™ Hospice & Palliative Care

At Evercare™ Hospice & Palliative Care, we provide compassionate care for those facing end-of-life issues and personal support to their families. Because patients prefer the comfort of familiar surroundings, we provide care wherever patients reside, whether in a private home, a nursing home, an assisted living facility or other in-patient setting.

Palliative care

Palliative care is for those who have advanced illness but are continuing curative therapies. Care includes physician consultation and support services for the patient and family. This can be a time to learn about how an illness is likely to progress and plan for end of life care.

Hospice

Hospice helps individuals live well. It is personal care that focuses on improving quality of life and reducing suffering. Individuals receiving hospice care typically have a life expectancy of weeks or a few months. Hospice provides services to support physical needs as well as the spiritual and emotional needs of individuals and their families and is tailored to personal needs and preferences.

Hospice services are provided regardless of ability to pay. In addition, Evercare Hospice remains dedicated to helping patients obtain insurance coverage for hospice services when appropriate. Medicare, Medicaid and most private insurance companies cover hospice-related services at 100 percent, typically not requiring a deductible or copayment.

Covered hospice services include visits from hospice team members, as well as medications, medical equipment and supplies related to the terminal diagnosis. We bill insurers directly, eliminating the need for patients to file claims or other paperwork for care related to the terminal illness.

Is hospice and palliative care an appropriate option?

It's never easy to know when the time is right, but you may want to discuss hospice and palliative care with your physician, if any or all of the following are present:

  • Progressive declining health despite treatment
  • Increased or uncontrolled pain
  • Frequent hospitalizations
  • Repeat or multiple infections
  • Progressive or profound weakness and fatigue
  • Shortness of breath with or without oxygen
  • Decreased ability to perform regular activities of daily living
  • Alterations in mental status
  • Exhausted patient and family or caregivers
  • Patient and family wants to stop aggressive treatments

Care and dignity


Our local program leaders work directly with the medical community, families and caregivers to help ensure that every patient is treated with the greatest respect and dignity.

Our staff serves as patient advocates and helps deliver sensitive, expert care using state-of-the-art techniques in pain management and symptom control. Physicians with experience in hospice and palliative medicine lead each hospice clinical team. Our hospice nurses have training and experience serving the unique needs of patients nearing the end of life.

And, through our established relationships with community physicians, hospitals and nursing homes, we have a unique understanding and compassion for the health issues facing hospice patients.
